Disciplinary and Criminal Activity Disclosures
Disciplinary Disclosure
Illinois State University is committed to maintaining a safe and positive environment for all members of the University community. Therefore, applicants who have been required to leave any educational institution (high school, college, or international equivalent) for disciplinary reasons are required to disclose this information as a mandatory step in the application process and are reviewed by the University prior to an admission decision being made. This does not include probation or dismissal for poor grades. Please note that such disclosure does not automatically disqualify an applicant.
If you answer yes to this question on the application, you will be required to provide the following information:
- Date of incident
- Institution where incident occurred
- Current status of institutional sanction
- An explanation of the disciplinary violation
- Upload institutional documents on institutional stationary that includes date, outcome, and disciplinary action taken
In addition, please address what you have learned from this experience, how this has affected your life, and what you will do differently if given the opportunity to enroll at Illinois State. Please note that we may need to request additional information including permissions for access to institutional records.
Applicants are responsible for verifying that all required information has been received by the University. Please contact the Office of Admissions if you have questions about your specific situation or the review process at (800) 366-2478 or (309) 438-2181.
