Disciplinary and Criminal Activity Disclosures

Disciplinary Disclosure

Illinois State University is committed to maintaining a safe and positive environment for all members of the University community. Therefore, applicants who have been required to leave any educational institution (high school, college, or international equivalent) for disciplinary reasons are required to disclose this information as a mandatory step in the application process and are reviewed by the University prior to an admission decision being made. This does not include probation or dismissal for poor grades. Please note that such disclosure does not automatically disqualify an applicant.

If you answer yes to this question on the application, you will be required to provide the following information:

  1. Date of incident
  2. Institution where incident occurred
  3. Current status of institutional sanction
  4. An explanation of the disciplinary violation
  5. Upload institutional documents on institutional stationary that includes date, outcome, and disciplinary action taken

In addition, please address what you have learned from this experience, how this has affected your life, and what you will do differently if given the opportunity to enroll at Illinois State. Please note that we may need to request additional information including permissions for access to institutional records.

Criminal Activity Disclosure

Illinois State University is committed to maintaining a safe and positive environment for all members of the University community. Therefore, applicants who have been convicted of, plead guilty to, or been placed on court supervision for any criminal offense (not including routine traffic offenses, juvenile proceedings, or officially expunged records) are required to disclose this information as a mandatory step in the application process. A previous conviction or current indictment requires review by the University prior to an admission decision being made. Please note that such disclosure does not automatically disqualify an applicant.

If you answer yes to this question on the application, you will be required to provide the following information:

  1. Date of conviction
  2. Location of conviction
  3. Court disposition (outcome and sentence, if applicable)
  4. A brief explanation of conviction
  5. Upload official court documentation during the application process.

Please note that we may need to request additional information.

Applicants are responsible for verifying that all required information has been received by the University and for maintaining a copy of the receipt certifying submission. Questions in regard to the information to be submitted or the review process should be directed to the Office of Admissions at (800) 366-2478 or (309) 438-2181.

Appeal Process

If you would like to appeal the decision to deny your admission based on information you provided during your application process, you can submit an appeal in writing to the Director of Enrollment Management at Admissions@IllinoisState.edu. The appeal should include the official reason for appeal along with any other information and/or documentation you wish to be considered. You must appeal within 14 business days from the date of your denial email. Please note, if an appeal is granted, timing and space limitations in a given semester may affect your ability to proceed with admission should a decision be overturned.
